The digital patient: wearables in chronic disease care

How will people with long term medical conditions access care in the future? As sensors, wearables and online platforms become part of the services offered by doctors and therapists, patients are now able to access care options at home. These digital care delivery models enable patients more control over their health and illness and may potentially save the cost of clinical visits.
